I was halfway through my second coffee when the phone buzzed. The caller ID read: icare Urgent Care OKC.
“Patient monitor’s down,” the facility manager said. “Not glitching. Down. We have 41 patients on today’s schedule, and the waiting room is already filling.”
This is what I do. I’m an emergency equipment specialist at icare, the person who gets called when a piece of medical equipment fails at the worst possible moment. We distribute everything from patient monitors and dental chairs to surgical instruments, imaging systems, mobility scooters, and ophthalmic devices—so “worst possible moment” happens a lot. Most urgent calls default to one of two answers: cheapest or fastest. This facility manager did something more useful—she asked what “refurbished” actually meant.
Here’s the honest answer. Certified refurbished is not the same as “used off Craigslist.” When we offer a certified refurbished patient monitor, it means the unit went through a full teardown, worn parts were replaced, electrical safety was tested, and calibration was verified against manufacturer specifications. It comes with a warranty. That’s different from a “pre-owned” unit that shows up in a box with no paperwork.
To be fair, refurbished equipment has a bad reputation for a reason. Some sellers slap “certified” on a unit after a visual cleanup, and buyers pay for that with downtime. That’s why it matters who did the refurbishment and what warranty comes with it.
The price difference was real: about $4,650 for the refurbished monitor versus $7,900 for a brand-new unit with a similar feature set. And with manufacturer wait times stretching to two or three weeks in March 2025, new wasn’t just more expensive. It wasn’t possible this week.
I recommended refurbished. She agreed. The monitor arrived before 1:30 PM, meaning zero canceled appointments.
That call stuck with me because she asked the question most buyers skip. She didn’t ask “what’s the cheapest option?” She asked “what’s the responsible option, given our context?” In my experience, that distinction separates good purchases from expensive regrets.
An informed customer asks better questions and makes faster decisions. I’d rather spend ten minutes explaining the difference between certified refurbished and unvetted used equipment than get a callback two weeks later when something fails.
A Las Vegas Dental Clinic Taught Me About Duty Cycles
At 9:45 AM, the phone rang again. This time it was an icare Dental Las Vegas location. Their CBCT unit was already down for scheduled service. And now the compressor running three operatories was making sounds no compressor should make.
The office manager was hoping for good news. What she had instead was a tempting deal: a “dental compressor” her owner found online for $450.
I get the appeal. To be fair, $450 looks very reasonable next to the $2,100 we usually quote for a continuous-duty compressor. But that price difference exists for a reason.
Dental compressors need a duty cycle that matches real practice demands. A three-chair office with a CBCT unit can run a compressor for extended periods throughout the day. A $450 oil-free hobby compressor might be rated for a 20–30% duty cycle, meaning it can run for a few minutes and then needs a long rest. Use it like a clinical compressor and the motor overheats—usually on a Monday, usually with a full schedule.
Based on our internal service records from Q4 2024, we logged 11 emergency calls involving failed dental compressors. Three of those units were bought online from sellers marketing them as “dental compressors.” They weren’t built for continuous clinical use.
I still kick myself for the one I didn’t catch early. In 2023, a clinic saved roughly $1,400 on a compressor purchase. The compressor failed within months. Between canceled appointments, emergency replacement, and lost production, the real cost was north of $4,000. I should have asked more questions about their volume before they ordered.

I don’t automatically defend our price. If the set meets the clinical requirement, the competitor quote is worth considering. But we went through the spec sheet line by line.
The competing set was missing critical components: the valve sizing kit, plus specialized forceps designed for minimally invasive valve procedures. On paper, the set looked comparable. In the operating room, it wouldn’t have been complete.
Heart valve replacement is not a procedure where you want to discover missing instruments after the patient is prepped. A delay isn’t an inconvenience—it’s a patient safety issue. The coordinator asked the right question: not “can you match the price?” but “is this genuinely the same set?”
That comparison saved a lot more than $9,000. Last year, a facility that tried the discount route paid $6,400 in emergency rentals and expedited shipping when they discovered their “complete” set wasn’t complete—two days before a scheduled valve case.
Cheaper, by itself, isn’t a red flag. Cheaper without a side-by-side spec review is.
The Mobility Scooter That Didn’t Fit Through the Door
By early afternoon, I was on a call with a discharge planner at a rehabilitation center. The question sounded straightforward: “We need a mobility scooter for a patient heading home.”
It wasn’t straightforward. The patient’s family had already bought a heavy-duty outdoor scooter. This thing was built for uneven sidewalks and long distances. Problem: it measured 32 inches at the widest point, and the patient’s apartment doorways were 30 inches.
The scooter never made it inside.
The discharge planner was frustrated—not with the family, but with the lack of guidance they’d had. A mobility scooter is not one-size-fits-all. Width matters. Turning radius matters. So does weight capacity, battery range, whether the patient can transfer independently, and whether the scooter can be transported to appointments.
By the time we finished the call, the family had a rental plan for the right type of scooter while they worked out a longer-term solution. The expensive outdoor scooter was returned. Not ideal, but better than forcing a patient to choose between staying inside and an aid that didn’t fit their home.
That call wasn’t an equipment failure. It was an information failure.
“How Does a Tonometer Work?” Is the Best Question I Heard That Day
The last call came at 2:15 PM. A clinic director at a growing eye care practice was starting a glaucoma screening program. She had quotes from three suppliers, but none of them explained the actual technology. So she asked me directly: How does a tonometer work?
Here’s the short version. A tonometer measures intraocular pressure—the pressure inside the eye—which is a key indicator for glaucoma risk. High pressure doesn’t always mean glaucoma, but it’s one of the most important screening signals we have. According to the CDC, about 3 million Americans have glaucoma, and many don’t know it yet, which is why accessible screening matters.
There are several ways to measure IOP:
- Applanation tonometry gently flattens a small area of the cornea and measures the force required. It’s often considered the reference standard in comprehensive eye exams.
- Non-contact tonometry (“the puff test”) uses a calibrated burst of air to applanate the cornea. No touching, fast, and easy for screening workflows.
- Rebound tonometry uses a very small, lightweight probe that briefly touches the eye and measures deceleration. It’s portable and often well tolerated.
Which one is right depends on the setting, the patient population, and who will be using it. An urgent care doing quick screenings might prefer non-contact. A glaucoma follow-up practice might need applanation-level precision. That’s not a one-line answer.
The clinic director didn’t buy anything that afternoon. But she left the conversation actually understanding the options. Six weeks later, she ordered the system that matched her clinic’s workflow—and she’s referred two other practices to us since.
Education isn’t a sales tactic. It’s the foundation of a good purchase.
The Real Lesson From a Chaotic Day
That Tuesday was chaotic. Five urgent requests, five different care settings, one connecting thread.
The people who asked questions walked away with better outcomes. The ones who acted on assumptions—or bought on price alone—ended up paying more in time, money, or stress.
I used to think my job was speed. Get the right equipment where it needs to be, fast. But in March 2025, I learned something different: speed doesn’t matter if the equipment doesn’t fit the actual need.
